85-year-old, male (R.G.) admitted from Medstar Georgetown University Hospital status post fall, Heart failure exacerbation and confusion. Patient has a history of TBI, Hemiplegia, AFIB and Diabetes.
Monitor Fluid Balance – Daily Weight Monitoring; weight trended down
Aspiration Precaution – Downgraded Mechanical Soft Diet; Diet upgraded
Monitor Labs – BBG’s, BMP and CBC
Medication Management – Lasix 40mg daily, Lantus, Asprin, Atorvastatin, Metoprolol
Wound Healing – Sacrum – RESOLVED
Upon admission, Patient required moderate assistance w moderate assist to stand, and standing tolerance as well as endurance was poor. He received occupational and physical therapy 5 times a week for 3 weeks. In addition, the Therapy Team worked with the patient and his wife to ensure safe process with car transfers. Upon discharge, patient is ambulating 150 feet with stand
by assist.
Patient returned home with his wife after a 21 day stay in Short Term Rehab He was a delicate patient to care for and was managed in house without any unplanned hospitalizations. He will be followed by Inova Home Health and will continue to follow with PCP, Dr. Ezequiel Molina in the community.
